Your Physical Therapy Partner for the Ambulatory Specialty Model
Better Partners. Better Outcomes. Better Spine Care.
Beginning in 2027, the CMS Ambulatory Specialty Model creates greater accountability for participating specialists caring for Medicare patients with low back pain—including a focus on functional outcomes, quality, cost, and coordinated care.
Stride Physio is prepared to partner with local physicians to help patients access high-quality conservative care quickly—and to provide meaningful information about their functional progress.
A Collaborative Physical Therapy Partner
Stride Spine Collaborative is designed to serve as a reliable extension of your care team by providing timely access to physical therapy, measurable functional outcomes, individualized orthopedic care, and communication that is genuinely useful to your practice.
Quick Access
New-patient openings every week
A conservative-care pathway only works when patients can access it. Our growing team allows us to get new spine patients started promptly.
Measurable Function
Oswestry Disability Index embedded in our EMR
We routinely use the Oswestry with our low back pain patients to objectively measure functional status and change over the course of care.
One-on-One Care
45-minute individualized visits
Patients work directly with their physical therapist throughout each visit, allowing for individualized examination, treatment, education, and progression.
Clinical Communication
We want to be an extension of your team
We can communicate functional progress, barriers to recovery, and concerns requiring further medical evaluation—not simply send a discharge note at the end of care.
Coordinated Care Centered on Function
Improve Function
Help patients return to work, recreation, exercise, and the activities that matter to them.
Support Conservative Care
Provide patients with an effective, evidence-based pathway for managing low back pain.
Coordinate Care
Keep physicians informed when patients are progressing— and when they are not.
Use Healthcare Resources Wisely
Help patients move through an appropriate care pathway while avoiding unnecessary escalation when conservative management is appropriate.
Tracking Functional Change in Low Back Pain
Objective functional measurement is built into our care process. The Oswestry Disability Index is embedded in our EMR and used with low back pain patients to follow functional change over the course of care.
In 2026, Stride has tracked Modified Oswestry data across 129 low-back-pain cases.
Internal Stride Physio quality-improvement data. Outcome reporting will continue to expand as additional follow-up measurements become available.
A Clear Conservative-Care Pathway
Make It Easy to Get the Right Patient Into Physical Therapy
